MAY 9

Hungry Eyes Jamaican Restaurant & Bar, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: raw fish incorrectly stored next to veggies and raw chicken in the reach-in; sauce cups prepped yesterday are not date marked; bulk buckets of white powder ingredients and squeeze bottles of oil are unlabeled.

Ming's Dynasty, 1200 North Concord Avenue – The following violations were reported during a routine inspection: temperature of cooked noodles made yesterday in prep top and walk-in is elevated; cooked egg rolls, chicken, pork and shrimp are not date marked; did not observe time marks on cabbage, chicken or garlic and oil; small containers of white substances on the cook line are unlabeled; a cut jug cannot be used as a scoop; racks in walk-in and dry storage are rusty; prep unit has a torn gasket; front counter has a hole; observed grease buildup on cook line equipment and the piping behind; observed buildup on racks inside and outside units and inside walk-in; walk-in fans are dusty; observed food and other dirt on racks by cook line and hand sink; walls behind cook line and by prep unit have food splatter; observed dirt buildup under units and in dry storage area.

MAY 10

America's Best Wings, 7536 West Broad Street – The following violations were reported during a routine inspection: an employee didn't use soap to wash his hands; an employee failed to wash hands after entering kitchen from outside and touching his shoes; observed dried food debris on both slicers on prep table; menu lists catfish but swai is being served; observed dust and food debris on wire racks, prep table shelves and fryers.

Cava Grill Short Pump, 11780 West Broad Street – The following violations were reported during a routine inspection: facility doesn't have a certified food protection manager; dried food particles found on clean dishes; chemical dishwasher only dispensed 10 ppm of chlorine after multiple priming attempts; temperatures of tomato and onion mix on the line are elevated; many items in the walk-in are improperly covered and stacked while cooling; pickled onions and sauce mixes incorrectly stored in large, deep containers; walk-in cooler fans are clicking loudly; wire racks in cooler are rusty; front vent on ice machine and walk-in cooler fan guards are dusty; gaskets have residues on them; prep area wire rack is coated in white residue (possibly cheese); three-compartment sink sprayer is hanging below the flood rim; wall behind ice machine is dirty; floor drains have buildup; floor around dishwasher is soiled; floor under and behind cook line is greasy.

MAY 11

Mezeh Grill, 6927 Forest Avenue – The following violations were reported during a routine inspection: there wasn't a person in charge at the facility; no employees could answer what temperatures are required for chicken; facility doesn't have written procedures for cleaning up vomit/diarrhea; open drinks found on shelf; temperatures of veggie kefta, lamb, carmelized egg plant and falafel on service line are too low; temperatures of kale and spinach are elevated; temperature of hummus made today is elevated because it wasn't cooled properly; pans are stacked while still wet.

MAY 12

Backwoodz BBQ, 400 West Williamsburg Road – The following violations were reported during a routine inspection: person in charge couldn't provide certified food protection manager certificate; food debris found inside and around microwave under prep table; temperatures of brisket, chicken BBQ and ribs are too low; no date marks found on mac n cheese, baked beans, brisket, ribs, pulled pork or chicken; single-use items are not stored inverted; observed grease and food debris on sides of fryers.

Rico's Mexican Grill, 11321 Nuckols Road – The following violations were reported during a routine inspection: orange residue found on shield inside ice machine; observed improper cooling methods (ground beef in deep pans while cooling and salsa in portioned cups with lids while cooling); observed dust and food debris on prep shelves storing food and single-service items.

Tropical Smoothie Café, 11321 Nuckols Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; observed heavy buildup of mold inside blender lids and cups; temperatures of three pans of bagged chicken in prep unit are elevated; an employee with fingernail polish or artificial nails is handling exposed food while not wearing gloves; an employee is wearing dangling bracelets; small prep cutting board is heavily scratched and stained; door gaskets on both prep units are torn; observed dust buildup on wire racks, bulk bin, inside reach-in cooler and outside surfaces of blenders; observed dried food debris on walls and floors.

Lebanese Bakery and Restaurant, 6215 West Broad Street – The following violations were reported during a routine inspection: employee drinks incorrectly stored next to equipment and on top a cutting board in kitchen; raw beef incorrectly stored next to cooked beef in reach-in unit; observed debris greater than one day old on multiple utensils in utensil container; temperature of rice in rice warmer is too low; temperature of bag of cheese in lowboy unit is elevated; air temperature of this unit is elevated; temperatures of tomatoes, lettuce, beef and spinach in prep top unit are elevated; air temperature of this unit is elevated too; to-go items don't have correct labels that list net weight, ingredients, allergens, etc.; foods are incorrectly stored directly on floor in dry storage room; reach-in unit has a torn gasket; air temperature of sandwich unit is elevated; cutting boards in kitchen are heavily scratched; ceiling vent and tiles in dish room are dusty; stain on wall in dish area; floor tiles in kitchen are cracked; ceiling light above drink cooler in dining area isn't working; ceiling light above pizza oven is cracked.

MAY 15

The Boathouse at Short Pump, 11800 West Broad Street – The following violations were reported during a routine inspection: facility doesn't have procedures for cleaning up vomit; shellstock tags are not kept in chronological order and did not have the date of the last shellstock served on them; raw burger meat incorrectly stored next to whole muscle beef and over shellstock in walk-in; raw shell eggs incorrectly stored next to ready-to-eat foods and above packaged liquids in walk-in; old sticker residues found on clean dishes; can opener and interior top surface of ice machine are dirty; potato cutter has old debris on it; bar menu is missing a consumer advisory for the tequila oyster shooter; knife incorrectly stored in crevice of make table; dishes on the drying rack are not inverted and still had standing water in them; racks in the walk-in and a few lowboys are rusty; plastic seafood containers are being washed and repurposed for other storage throughout kitchen; surfaces of cook line equipment have food residues on them; drip trays and dry storage racks are dirty; top of expo shelving and top of spice shelves in prep area feel sticky/gritty; walk-in cooler and beer cooler fan guards are dusty; floor under cook line is greasy; white residue observed behind the ovens on the cook line; wall behind dishwasher is dirty.

Wing Zone, 8310 Staples Mill Road – The following violations were reported during a routine inspection: all hand sinks are missing paper towels and hand soap; container of flour used to batter raw chicken is heavily encrusted with dried food debris; raw chicken is stored on a table where ready-to-eat orders are prepared and next to bread on prep table; a heavily soiled wiping cloth is stored on prep unit cutting board; observed dried food debris and grease buildup on wire storage racks, fan systen in walk-in cooler, fryers, prep table shelves, hood filters and under grill; observed buildup of grease and food on floors, under cook line and three-compartment sink, inside walk-in cooler and on walls; several light fixtures are missing shields.

MAY 16

Little Angela's, 10246 Staples Mill Road – The following violations were reported during a routine inspection: an employee is handing deli meat with bare hands; the date when the last shellstock from the container was sold or served is not recorded on the tag or label; sanitizer concentration in dish machine is too low; gaskets on reach-in freezer and salad prep unit are torn; wire racks in the walk-in are rusted; prep cutting boards are heavily scratched and stained; observed heavy dust and food debris buildup on wire storage racks and blue dishwash racks; there are areas of low grout on the floor; ceiling vents/panels are dusty.
